| Who is eligible for Medicare? |

A patient must be homebound: a condition
due to illness or injury which restricts his/her ability to
leave his/her place of residence except with the aid of
supporting devices or if he/she has a condition which is such
that leaving his/her home is medically contraindicated.

Or, under physician’s direction, referred
by a family member, physician, facility, hospital or community
sources.
